Kept current, not left to go stale.
Capacity by service and area to be published here once confirmed, and kept current rather than left to go stale. A referral we decline quickly is more useful to you than a placement that fails at month three.
-
The participant's name and preferred contact, and whether we should approach them or you first
-
Plan management type, and the relevant plan dates if you have them
-
Which supports you are seeking and roughly what hours
-
Suburb or area
-
Communication preferences, access needs, or any preferences about workers
-
Risks or clinical considerations we should know before a first meeting
-
Whether anyone else is already providing support, and what
A decline arrives quickly.
Acknowledgement and response timeframes to be published once confirmed. Nothing will be stated here that Firstlight cannot meet every time.
The shape of it: we acknowledge the referral, tell you honestly whether we can take it, and come back to you either way. A decline arrives quickly rather than being left to drift.
We will raise issues early rather than at review.
Declining engagement, a support that is not landing, a goal that has stalled, or a participant whose needs have moved beyond what we should be providing.
Where a participant would be better served elsewhere, you will hear that from us. Holding a placement that is not working helps nobody, least of all the participant, and it costs us your confidence for the next referral.
Reporting and documentation.
Reporting formats, frequency and what we can provide for plan reviews to be published once confirmed.
Ask first
Send a referral, or ask first.
A quick call about whether we are a fit is usually faster than a formal referral that comes back declined.
